A form used to verify members’ health care appointment(s) as part of the reimbursement process. Please see the Reimbursement Guide for more information about the reimbursement process.

Similar to the Appointment Verification form, this form is used to verify members’ health care appointments that happen at the same location on a consistent and reoccurring basis.

Provides a list of expectations members can expect of the Ride to Care program, and in return what the Ride to Care program expects of members.

Online form to submit a grievance and/or other feedback about the Ride to Care program. Online submissions are monitored from 7 a.m. to 6 p.m., Monday through Friday. For urgent grievances please call Ride to Care directly at 503-416-3955 for immediate assistance.
