Riders Guide

Provides helpful information for members and their supports about how the Ride to Care program works.

Appointment Verification Form

A form used to verify members’ health care appointment(s) as part of the reimbursement process. Please see the Reimbursement Guide for more information about the reimbursement process.

Ride Subscription Verification Form

Similar to the Appointment Verification form, this form is used to verify members’ health care appointments that happen at the same location on a consistent and reoccurring basis.

Provides a list of expectations members can expect of the Ride to Care program, and in return what the Ride to Care program expects of members.

Appointment of Representative Form

A form used to designate an authorized representative to navigate any part of the NEMT process on behalf of a member

Secure Transport Consent Form

This form is used to provide written consent for secure transport, when required.

Grievance and Feedback Form

Online form to submit a grievance and/or other feedback about the Ride to Care program. Online submissions are monitored from 7 a.m. to 6 p.m., Monday through Friday. For urgent grievances please call Ride to Care directly at 503-416-3955 for immediate assistance.

Ride Cancellation Form

Ride to Care has sunset the use of this form. Please call Ride to Care at 855-321-4899 or 503-416-3955. When you call, select option 1 to leave a voicemail to cancel a trip, or remain on the line to speak to a representative.
